a list of issues that you would like to work on in
counseling.

you have any extremely painful incidents or traumatic events
that may be part of the psychological basis of your current
difficulties, please make a brief list of them as well.
- By
preparing in writing some of your therapeutic goals and
organizing your thoughts regarding what you need to tell me,
we will be ready to make the best use of your time in
